    Pressure works on the fabric as long as there is an equal and opposite pattern. The mesh density for edged area's you may want to puff out need to be a suitably small enough mesh particle size so the cloth mesh can flex in at least 3 points to form an arc > therefore mesh density in a region may need to be increased locally for that detailed area to puff out.

     

    Finally your fabric choice in preset needs to be soft + stretchy  enough as a fabric to allow the material to expand under pressure, which you may need to increase. Basically both the mesh and fabric follow the same physical laws of real life - so time to dust off that high-school physics class you maybe missed and apply that theory to your garment construction and model parameters.
