How do I get the hole of the letter and the letter at once as an internal shape on the T-shirt as an application
I can only get the outline of the letter

You will need to sew the D onto the internal lines of the shirt. We have a beginner series HERE which you might find helpful. this video HERE specifically goes over patches and pockets which may help to answer this specific questions

https://support.clo3d.com/hc/en-us/profiles/387858256974-viauniversity, I didn't try, is the issue that you can't trace the outline of a hole? (I think that's happened to me.) It might help to draw an internal line around the hole on the letter first. Copy that and paste onto the garment. Then delete the internal line from the applique (inadvertently sewing an internal line that coincides with a pattern outline will cause problems). Then sew both outlines to the garment. Layering the letter under the garment, will allow you to get the outlines positioned correctly.
