I would create the panels and then with internal lines create one of the pattern pieces. Then copy and paste that line at the necessary intervals. Use the same lines to cut and sew the pieces so you can apply pressure only to the pieces where you want to see the quilt. In my example the brown pieces do not have pressure applied to it. Make sure to turn off the 3D seamline intensity so it looks flat.
I'd then Layer Clone under that same pieces so you can create the quilt. Apply pressure to both pieces (not the brown ones) and you should have the effect.
I would do this once the whole garment is assembled.

I will show you a process that does this prior to the garment assembly, it's a process for quilting that I have used for years that gives a range of crease frequencies and a superior result to many of the CG visuals for quilted coats out there.
It's important to do this before you assemble the garment so you also get the variation in the texture mapping so it matches the final pattern shape. You cannot do this after the assembly as the frequencies in the creasing are technically from different garment pose positions and you want to have some control over this aspect. So I never do this on an assembled garment for the primary crease and pucker as these are additive and not singular frequencies. You may also need to run 3-4 additional crease frequencies out for the garment, which is impossible once the garment is assembled.
Will post an example with the process and tools. Takes a few minutes to do. But gives a stella result, and allows you to control the look of the final render with more values.
